COVID-19 Update

Dear Autumn Care Families,

Thank you all so much for the patience you’ve displayed as we continue to navigate these difficult and confusing times. Over the last few months the increasing vaccination rates and declining COVID-19 caseloads in Cache county have allowed the reopening of many areas of our community. We have loved seeing all the families and visitors back in our buildings. Each of you bring a unique light and vitality to life at Autumn Care. Unfortunately, our county has crossed a threshold that requires Autumn Care, as a strictly regulated healthcare facility, to take a few precautionary steps in order to reduce the risk that COVID-19 enters our buildings. Those steps follow:

  • Testing each person (this includes staff, residents, and visitors regardless of vaccination status)  that comes inside our facility at least weekly for COVID-19.

  • Ensure proper face coverings are worn by all staff and visitors while in the building and not socially distanced.

  • Practice excellent hand hygiene. For visitors this will mean washing or sanitizing their hands upon entering the building. Our staff will continue to wash/sanitize their hands frequently, especially when shifting between activities, before and after assisting residents, and anytime they are working with food. 

  • Maintain a record of all visitors to the building along with a few pieces of COVID-19 related information.

The above steps are licensure requirements for Autumn Care when the COVID-19 positivity rate in Cache county is above 10%. As of Thursday, July 29, 2021 the Cache county positivity rate is 10.63%. We will monitor and review this rate every week and adjust our procedures in accordance with changes in our county’s positivity rate and as required by local and state health departments. Family visits are incredibly important to the mental well being of our residents, their family members, as well as our staff. At this time we are still encouraging families to visit as often as they wish. During the times when our weather is comfortable we would love for you to utilize our patios during your visits. While outside you may choose to remove your mask. 

By adhering to the required steps it is our sincere belief that we can keep this confounding disease from entering our facility and avoid interruptions to family visits.

ADDITIONAL INFORMATION REGARDING COVID-19 PROTOCOLS

Testing
Our nurses will hold regular testing times each week (below). Before visiting please text the nurse scheduled on your preferred day and let them know you would like to be tested. If you have not previously tested with us please fill out this form. It only needs to be completed one time. 

If you are unavailable during the scheduled testing times please contact Kandice to make alternate arrangements.

Scheduled COVID-19 Testing Times:

  • Monday 11am-12pm

  • Wednesday 11am-12pm

  • Thursday 1pm-2pm

Testing Procedure:

  1. Remain in your vehicle.

  2. Call the facility at (435) 563-3333 to let them know you have arrived and where you parked.

  3. The nurse will come to your vehicle to perform the test.

  4. Please remain in your vehicle for 15 minutes while the test processes.

  5. After the results come back negative you are clear to visit Autumn Care for the following seven days.


Facility Check-in

Upon entering the building, there will be a QR Code or a sign in sheet. Please sign in as you arrive and use the provided hand sanitizer.

The questions you will be asked are:

  1. Do you have Signs or symptoms of a respiratory infection, such as fever, cough, shortness of breath, or sore throat?

  2. Have you been around anyone that has had any respiratory signs or symptoms such as fever, cough, shortness of breath or sore throat?

  3. Have you had Contact, within the last 14 days, with someone who has a confirmed diagnosis of COVID-19,i s under investigation for COVID-19, or has a respiratory illness?

  4. Have you or anyone you have been in contact with traveled internationally within the last 14 days?

  5. Do you Reside in a community where community-based spread of COVID-19 is occurring?

  6. Have you or anyone you’ve been in contact with taken any recent trips (within the last 14 days) on cruise ships or participated in other settings where crowds are confined to a common location?

If the answer to any of these questions is yes, we ask that you postpone your visit until you can respond ‘no’ to all the above questions.

Please help us to keep our residents, your loved ones, healthy and happy. Remember to wear masks in the common areas and practice excellent hand hygiene. Masks are available at the front doors.
